Six Month Program Details

you are not only a beautiful woman on the exterior but also, and more importantly on the inside and would enjoy assisting you in setting and achieving your health and wellness goals.

I have attached a required health history form for your completion. Please  subscribe to receive required health history questionnaire form for completion, and/or email me at – madeleinebydesign@gmail.com.  You may either scan and email it back to me or hold onto it and I will pick it up and make time to discuss your responses next week.

As a Health Coach, some of the services I offer within my 6-month program, include:

  • Personal mentoring and motivation counseling (12 sessions)
  • A personalized exercise regimen/program
  • 1 personal training session at a gym (more available at increased monthly rate)
  • Shared recipes
  • At-home cooking class
  • Informational handouts and health-related and motivational emails
  • A pantry make-over
  • Two shopping tours to a Farmers Market and/or Wholefoods/ Health food & supplement store

My initial consultation, at which time we will discuss your goals and health at length, is complimentary.  We would, aside and apart from the “field trips/outings”, schedule to meet  once or twice a month (depending on client need) for 6 months and discuss your goals and achievements moving forward.
